Our Connect initiative improves access to responsive and equitable care and services to address disparities in health and well-being. We support community-based organizations that provide quality, coordinated resources and supports that adapt to the changing priorities and needs of diverse communities.
Our Priorities
Our priorities include:
- Delivering culturally, linguistically, racially, and ethnically appropriate care and services that address physical, mental, social, and economic drivers of health and well-being;
- Increasing health insurance and public benefits coverage;
- Providing case management and navigation assistance.
