Community Voice and Leadership

The Philadelphia Health Partnership strives to actively engage Philadelphians in designing and implementing systems, policies, and programs that advance health equity. In doing so, we support the delivery of quality care and services that are responsive to individual and community preferences, values, and needs.

Context

Community voice and leadership are cross-cutting components of the foundation’s strategic initiatives, reflecting our belief that all people should be actively engaged in decisions that affect their health and well-being. Across our portfolio, foundation grantees provide substantive opportunities for their constituents to participate in the design, implementation, and adaptation of care and services. At the policy and systems level, the foundation provides additional dedicated support for organizations that focus on mobilizing and amplifying community voice and leadership to advance health equity as a core aspect of their mission.

2. Outreach, education, and advocacy to assist participants and providers in understanding and navigating Pennsylvania’s shift to mandatory managed care for long-term services and supports (Community HealthChoices). Current grantees:

  • Center for Advocacy for the Rights and Interests of the Elderly (CARIE)
